Travel Physical Therapist in Lakeland, FL

A Travel Physical Therapist will provide home health therapy services to adults and geriatrics in Lakeland, Florida on a 20-week contract. The caseload includes post-surgical, orthopedic, neurological, cardiac, respiratory, and mobility-related needs for skilled visits in the home setting.

• Complete home health assessments and reassessments
• Develop and update individualized plans of care and home exercise programs
• Provide gait training, strengthening, and mobility support
• Teach patients and families safe use of walkers, lifts, and other assistive devices
• Monitor vital signs, pain, skin integrity, and functional progress
• Document visits and coordinate care with the interdisciplinary team

• Physical Therapist license in Florida
• BLS certification
• 1 year of home health experience
• Valid driver’s license and access to a vehicle
• OASIS training
• Experience with adult and geriatric patients

• OASIS experience
• Teaching hospital experience
• Community hospital experience
• LTAC experience
