Travel Pharmacist in Zanesville, OH

A Travel Clinical Pharmacist will support inpatient medication therapy in an acute care hospital on a 13-week contract in Zanesville, Ohio. The hospital is a Level 3 trauma setting.

• Review medication orders for safety and appropriateness
• Prepare and verify inpatient prescriptions and therapy plans
• Assist with vancomycin dosing and warfarin management
• Monitor drug therapy and recommend adjustments
• Work with the care team on medication-related questions
• Maintain accurate pharmacy documentation

• Active Ohio Pharmacist license
• 1 year of inpatient hospital pharmacy experience
• Experience with vancomycin pharmacokinetic dosing
• Experience with warfarin management
• Strong inpatient medication order review skills
