Travel Pharmacist in Visalia, CA

A Travel Outpatient Registered Pharmacist will provide outpatient medication review, verification, and dispensing in a hospital setting on a 13-week contract in Visalia, California.

• Review prescriptions for appropriateness and safety
• Validate and dispense medications, supplies, and equipment orders
• Consult with providers and care teams to optimize therapy
• Answer medication questions from providers and staff
• Monitor therapy and document interventions in the medical record
• Provide patient-specific dosing, monitoring, and care coordination
• Support medication education and patient counseling

• Current California Registered Pharmacist license
• Doctor of Pharmacy degree from an accredited program
• Three years of pharmacist experience
• PGY-1 residency completion or at least two years of clinical pharmacist experience
• Strong documentation and collaboration skills
