Travel Pharmacist in Fredericksburg, VA

A Travel Inpatient Pharmacist will support medication reconciliation, discharge medication management, and order review in a short-term acute care hospital on a 13-week contract in Fredericksburg, Virginia. Epic charting is required, the unit is a busy Level II trauma center with a 330-360 census, and first-time travelers are welcome.

• Perform medication reconciliation for admissions and discharges
• Review orders and recommend therapy changes to the care team
• Support discharge medication management and follow-up planning
• Participate in interdisciplinary care rounds
• Counsel patients and families on medications and safety
• Prepare IV medications and maintain sterile technique

• Virginia pharmacist license in good standing
• Doctor of Pharmacy degree or Bachelor of Science in Pharmacy
• 2 years of inpatient or acute care pharmacy experience
• Epic charting experience
• Medication reconciliation, IV medication, and sterile technique skills

• Pyxis and Carousel experience
• Ambulatory care experience
• Anticoagulation, renal, and warfarin dosing
• Pediatric, geriatric, and pharmacokinetic dosing
