Travel Home Health RN in Portland, OR

A Travel Home Health Registered Nurse will provide skilled in-home nursing care in the home health setting on a 13-week contract in Portland, Oregon. The caseload includes adults and geriatrics, and documentation uses Epic and OASIS. First-time travelers are welcome.

• Assess patients in their homes and update care plans
• Perform wound care and wound vac management
• Manage PICC lines, G-tubes, ostomy care, and catheter care
• Educate patients and families on disease management and self-care
• Document visits in Epic and OASIS
• Coordinate interpretation support and communicate with the care team

• Oregon Registered Nurse license
• BLS certification
• 1 year of skilled home health experience
• Adult home health experience
• Epic charting experience
• OASIS documentation experience
