Travel Home Health RN in Le Mars, IA

A Travel Home Health Registered Nurse will provide in-home nursing care, assessments, and care planning on a 13-week contract in Le Mars, Iowa. The role serves patients in their homes and uses OASIS for documentation and care planning. Epic experience is a plus.

• Assess patients in their homes and update the plan of care
• Complete OASIS assessments and related documentation
• Coordinate care with providers and team members
• Educate patients and families on home care plans
• Monitor patient status and report changes promptly
• Support safe and effective home health visits

• Iowa Registered Nurse license upon acceptance
• 3 years of home health experience
• OASIS assessment and care planning experience
• Home care documentation and patient education skills

• Epic experience
