Travel Home Health RN in Grand Junction, CO

A Travel Home Health Registered Nurse will provide skilled nursing care in patients’ homes on a 13-week contract in Grand Junction, Colorado. The role includes one-on-one visits, comprehensive assessments, patient and family education, and documentation in an EMR.

• Complete in-home nursing assessments
• Provide skilled nursing interventions within RN scope
• Develop and update individualized plans of care
• Monitor changes in patient condition and report concerns
• Teach patients and families about medications, treatments, and self-care
• Document each visit in the EMR
• Coordinate care with interdisciplinary team members

• Active Colorado Registered Nurse license or active multistate compact license
• BLS/BCLS
• Valid driver’s license, reliable transportation, and auto insurance
• Strong assessment and critical thinking skills
• Ability to work independently in the field
• Comfort with documentation and EMR systems

• Home health experience
• HCHB experience
