Travel Case Management RN in Medford, OR

A Travel Case Management Registered Nurse will coordinate safe discharge planning for adult and geriatric patients in a hospital setting on a 13-week contract in Medford, Oregon. Epic charting is used in this acute care hospital.

• Coordinate safe discharge plans for adult and geriatric patients
• Collaborate with patients, families, and the care team
• Arrange durable medical equipment, home health, hospice, and post-acute placement
• Work with community partners and insurers to support transition planning
• Assist with referrals and discharge needs across inpatient units

• 1.5 years of case management experience in an acute hospital setting
• Care coordination and discharge planning skills
• CMS, HIPAA, and OSHA knowledge
• Epic charting system experience
• Acute care hospital experience

• BLS
• Utilization review concepts, including admission criteria, continued stay reviews, appeals and denials, DRG, and medical necessity
• Experience with medical-surgical, ICU, observation, or surgical services
• Knowledge of The Joint Commission, Core Measure, and National Safety Goals
