Travel Case Management RN in Anchorage, AK

A Travel Case Management Registered Nurse will coordinate discharge planning for patients in an acute hospital in Anchorage, Alaska on a 26-week contract. The unit uses Epic and serves neonates through geriatrics. The hospital is Magnet designated and Level II Trauma.

• Assess patient needs and discharge barriers
• Coordinate discharge planning and care transitions
• Review continued stay and medical necessity
• Collaborate with providers and social services
• Arrange post-acute services and follow-up needs
• Document case management interventions in Epic

• Active Alaska Registered Nurse license
• 2 years of acute care hospital experience
• Epic charting experience
• Care coordination, discharge planning, and utilization review skills

• BLS certification
• ACM-RN certification
• InterQual and Milliman guideline experience
• Prior authorization and continued stay review experience
