Travel Case Management RN in California

A Travel Case Management Registered Nurse will coordinate care and discharge planning in an acute care hospital on a 13-week contract in Burbank, California. It supports adults and geriatrics with computerized documentation in Epic, a case load up to 25, and welcomes first-time travelers.

• Assess discharge needs and care barriers for adult and geriatric patients
• Coordinate care plans with the interdisciplinary team
• Review medical necessity, admission criteria, and utilization needs
• Arrange discharge resources, DME, and follow-up services
• Document case management activity and plans in Epic
• Support disease management and plan-of-care updates for a case load up to 25

• California Registered Nurse license upon start date
• 2-year acute medical care manager experience in care coordination and discharge planning
• Epic charting system experience
• Acute hospital experience with admission criteria, medical necessity review, CMS, DRG, CPT, Department of Health requirements, HIPAA, and plan of care
• Experience in ICU, MS, and SDU/PCU/IMC/Obs settings
• Prior travel experience
