Travel Case Management RN in Lodi, CA

A Travel Case Management Registered Nurse will coordinate patient care and discharge planning in an acute care hospital in Lodi, California on a 13-week contract. The role supports a defined patient population from pre-admission through post-discharge and works with the care team to improve outcomes.

• Coordinate case management for patients throughout the hospitalization
• Collaborate with nurses, physicians and the multidisciplinary team
• Serve as a liaison for patients and families
• Guide discharge planning and continuity of care
• Monitor progress toward care goals and revise plans as needed
• Support reporting for the case management program

• Current California Registered Nurse license
• Graduation from an accredited nursing program
• Care coordination and discharge planning skills
• Interdisciplinary communication and patient advocacy skills
