Travel Case Management RN in Petaluma, CA

A Travel Case Management Registered Nurse will coordinate adult and geriatric care in an acute care hospital for a 26-week contract in Petaluma, California. The unit serves an average daily census of 30 to 35 and uses Epic.

• Coordinate discharge planning and care transitions
• Apply InterQual criteria to support utilization review
• Collaborate with social services and therapy teams
• Assess patient needs and arrange post-acute services
• Communicate care plans with the interdisciplinary team
• Document case management activity in Epic

• California Registered Nurse license
• 2 years of acute care case management and discharge planning experience
• Acute care hospital case manager experience
• InterQual criteria and utilization review experience
• Epic charting system experience
• Prior travel experience

• Ambulatory care experience
• Emergency Department experience
