Travel Case Management RN in Petaluma, CA

A Travel Case Management Registered Nurse will coordinate discharge planning and utilization review in an acute care hospital for a 13-week contract in Petaluma, California. The unit serves adults and geriatrics with a daily census of 30 to 35, and Epic charting supports InterQual-based case reviews.

• Coordinate discharge planning and care transitions
• Apply InterQual criteria for utilization review
• Review admissions and ongoing stays for medical necessity
• Collaborate with physicians and support services
• Communicate plans and resources with patients and families
• Document case management activity in Epic

• California Registered Nurse license
• 2 years of acute care case management and discharge planning experience
• Acute hospital case management background
• Epic charting experience
• Prior travel experience
• InterQual criteria, admission criteria, CMS guidelines, HIPAA guidelines, and medical-surgical and ICU experience

• Ambulatory care experience
• Emergency Department experience
