A Travel Case Management Registered Nurse will coordinate care, discharge planning, and transitions in a hospital unit on a 13-week contract in West Hollywood, California. The role supports adolescents, adults, and geriatrics, with documentation in Epic and collaboration with social services and transportation support.

• Coordinate daily case management for diverse patient ages
• Assess discharge needs and plan safe transitions
• Collaborate with social services and transportation support
• Review care coordination and utilization needs
• Document plans and progress in Epic
• Communicate with the care team to support discharge

• Active California Registered Nurse license
• Basic Life Support certification
• 2 years acute care case management experience, including California case management experience
• Care coordination, discharge planning, and plan of care skills

• Epic charting system experience
• InterQual and Milliman guidelines familiarity
• CMS, DRG, and ICD-10 coding familiarity
• Teaching or community hospital experience
