Travel Case Management RN in Eureka, CA

A Travel Case Management Registered Nurse will coordinate care and discharge planning in an acute care hospital case management unit for a 13-week contract in Eureka, California. The role supports adolescents, adults, pediatrics, and geriatrics and works with social services. Epic charting is used.

• Coordinate care plans across the acute stay and discharge process
• Assess coverage needs, benefits eligibility, and utilization issues
• Review admission criteria, appeals, and denials
• Support discharge planning, DME needs, and protective services processes
• Collaborate with social services and interdisciplinary teams
• Document case management activities in Epic

• California Registered Nurse license
• 2 years of acute hospital case management experience with care coordination and discharge planning
• Epic charting system experience
• Prior travel experience
• Knowledge of CMS guidelines, HIPAA, and The Joint Commission standards

• DRG knowledge
• Prior authorization experience
• Workers' comp case management
