Travel Case Management RN in Mission Hills, CA

A Travel Case Management Registered Nurse will coordinate care and discharge planning in an acute care hospital on a 26-week contract in Mission Hills, California. The unit supports adolescents, adults, and geriatrics, uses Epic, and works with a 1:20 ratio. First-time travelers are welcome.

• Assess patients to support care coordination and discharge planning
• Apply InterQual criteria to support admission review and utilization needs
• Coordinate discharge plans with social services, transportation, and community resources
• Arrange DME, SNF placement, and ambulance transport as needed
• Participate in family meetings and treatment planning discussions
• Support patients with substance use, mental health, and housing-related needs

• California Registered Nurse license
• 2 years of acute care hospital case management experience with care coordination and discharge planning
• Epic charting system experience
• Prior travel nursing experience
• Knowledge of CMS guidelines and plan of care development

• DRG knowledge
• InterQual criteria experience
