A Travel Case Management Registered Nurse will coordinate care and discharge planning for adults and geriatrics in an acute care hospital in Downey, California on a 13-week contract. Allscripts charting is used, and first-time travelers are welcome.

• Coordinate care for adult and geriatric patients
• Complete needs assessments and discharge planning
• Develop and update plans of care
• Arrange durable medical equipment and resources
• Collaborate with the interdisciplinary care team
• Support disease management and transition planning

• California Registered Nurse license
• 2 years of case management experience in an acute hospital
• Care coordination, discharge planning, disease management, needs assessment, and plan of care skills
• Allscripts charting system experience
