Travel Case Management RN in West Hollywood, CA

A Travel Case Management Registered Nurse will coordinate care and discharge planning in a hospital case management unit on a 13-week contract in West Hollywood, California. The unit supports adolescents, adults, and geriatrics, uses Epic charting, and works with social services and transportation.

• Coordinate care plans across the interdisciplinary team
• Complete discharge planning and transition support
• Review admissions, continued stay, and utilization needs
• Collaborate with social services and transportation resources
• Document case management interventions in Epic
• Support care coordination for adolescents, adults, and geriatrics

• Active California Registered Nurse license
• Basic Life Support certification
• Minimum 3 years of acute care case management experience
• Previous California experience
• Care coordination, discharge planning, and plan of care skills

• Epic charting system experience
• Utilization review guideline familiarity
• Teaching or community hospital experience
