Travel Case Management RN in Portland, OR

A Travel Case Management Registered Nurse will coordinate discharge planning and care management for adults in an acute care hospital in Portland, Oregon on a 13-week contract. The unit serves a daily census of about 320, uses Epic, and manages a caseload near 20.

• Coordinate discharge planning and case management
• Assess patient needs and develop plans of care
• Collaborate with social services and care teams
• Review medical necessity and authorization needs
• Arrange DME and post-acute services
• Monitor progress and update documentation

• Oregon Registered Nurse license
• BLS certification
• 2 years of acute hospital case management and discharge planning experience
• Epic charting system experience

• Case management certification
• Appeals and denials experience
• Home health, hospice, or skilled nursing transition experience
• Medical necessity review experience
