Travel Case Management RN in Everett, WA

A Travel Case Management Registered Nurse will coordinate discharge planning and care coordination in an acute care hospital on a 13-week contract in Everett, Washington. Epic charting supports work in a Level II trauma center. The role focuses on patients needing next-level care coordination.

• Coordinate discharge plans and care transitions
• Assess patient needs and barriers to discharge
• Collaborate with nurses, physicians, and social services
• Arrange DME, services, and follow-up care
• Document care coordination in Epic
• Support utilization review and length of stay management
• Educate patients and families on next steps

• Current Washington Registered Nurse license
• BLS certification
• 1 year of acute care case management experience
• Acute hospital discharge planning and care coordination experience
• Epic charting experience
• Prior travel experience
• Assessment, plan of care, and DME ordering skills

• AIDA EMR experience
• Emergency Department case management background
• ICU case management background
• Med Surg case management background
