Travel Physical Therapist in White River Junction, VT

A Travel Physical Therapist will provide home health visits, evaluate and treat patients, and coordinate care in White River Junction, Vermont on a 16-week contract.

• Evaluate patients and develop individualized treatment plans
• Re-evaluate progress and adjust therapy plans as needed
• Carry out treatment programs in accordance with care standards
• Coordinate and communicate with the interdisciplinary care team
• Document evaluations, goals, objectives, and treatment responses
• Teach patients and family members treatment rationale and home care guidance

• Bachelor of Science in Physical Therapy
• Physical Therapist license in Vermont and New Hampshire, or unencumbered compact license
• 1 year of physical therapy experience in home healthcare
• BLS/BCLS certification
• ASHA
• CHT
• ACP Training
