Travel Physical Therapist in Rutland, VT

A Travel Physical Therapist will provide skilled therapy in a home health provider setting on a 13-week contract in Rutland, Vermont. The role focuses on OASIS assessments, individualized care planning, patient education, and coordinated home-based care.

• Complete comprehensive PT and OASIS assessments
• Develop individualized plans of care and functional goals
• Provide skilled treatment, teaching, and progress monitoring
• Coordinate care with physicians, nurses, caregivers, rehab staff, and students
• Supervise PTAs, PT students, and home care aides
• Document visits, required forms, and patient outcomes accurately
• Carry out wound care, staple or suture removal, and health checks as assigned

• Vermont Physical Therapist license upon acceptance
• Minimum 2 years of Physical Therapist experience
• Graduation from an accredited physical therapy program
• OASIS documentation and home health care coordination skills
