Travel Physical Therapist in Medford, OR

A Travel Physical Therapist will provide home health rehabilitation care on a 13-week contract in Medford, Oregon. Epic/Oasis documentation supports 1:1 visits with up to 6 patients per day.

• Evaluate mobility, strength, and home safety
• Develop and update treatment plans
• Guide therapeutic exercise and gait training
• Educate patients and families on home programs
• Document visits in Epic/Oasis
• Coordinate care with the clinical team
• Monitor progress and adjust interventions

• Oregon Physical Therapist license or pending licensure
• BLS certification
• 1 year of skilled home health experience
• Charting system experience
• Acute care, rehabilitation hospital, and skilled nursing facility experience; body mechanics, home exercise program instruction, patient and family education; musculoskeletal, orthopedic, neurologic, and post-op rehabilitation including cervicalgia, low back pain, spinal conditions, total hip and knee replacement, multiple sclerosis, muscular dystrophy, Parkinson’s disease, post-stroke rehabilitation, spinal cord injury, and traumatic brain injury
