Travel Physical Therapist in Mount Angel, OR

A Travel Physical Therapist will provide home health care in Mount Angel, Oregon on a 13-week contract. The role serves adults and geriatrics with one-to-one visits and up to 6 patients per day, using Oasis and Epic charting. First-time travelers are welcome.

• Evaluate patients and establish plans of care
• Deliver therapeutic exercise and mobility interventions
• Monitor progress and adjust treatment as needed
• Educate patients and caregivers on home programs
• Document visits in Oasis and Epic
• Coordinate care with the interdisciplinary team

• Oregon Physical Therapist license
• 1 year of skilled home health experience
• BLS certification
• OASIS documentation experience
• Geriatrics and orthopedics experience

• Epic charting system experience
