Travel Physical Therapist in Newport, VT

A Travel Physical Therapist will provide home health and hospice therapy visits on a 13-week contract in Newport, Vermont. The role uses HealthWyse in an outpatient care setting and covers patients seen in home visits.

• Evaluate mobility, strength, balance, and functional status
• Develop and update physical therapy plans of care
• Provide therapeutic exercise, gait training, and mobility education
• Document visit findings and progress in the EMR system
• Coordinate care with nursing and other team members
• Complete OASIS-related documentation as needed

• Active Vermont Physical Therapist license
• Minimum 2 years of physical therapy experience
• Home health, hospice, and OASIS experience
