A Travel Physical Therapist will provide rehabilitation care in a skilled nursing facility on a 13-week contract in Sacramento, California. The setting includes in house rehab services, a supportive team, and a varied caseload focused on helping residents regain independence.

• Evaluate residents and develop individualized treatment plans
• Provide therapeutic exercise, mobility training, and functional rehabilitation
• Assist with gait, transfer, and balance training
• Monitor progress and adjust care plans as needed
• Document treatment sessions and patient response accurately
• Collaborate with the rehab team to support continuity of care

• Active California Physical Therapist license
• Graduation from an accredited Physical Therapy program
• Strong clinical reasoning, communication, and documentation skills

• Skilled nursing facility experience
