A Travel Physical Therapist will provide rehabilitation and wound care services in a nursing home for a 13-week contract in Haverhill, Massachusetts. The role uses standardized assessments and evidence-based treatment for residents with physical functioning disorders and mobility needs.

• Screen and evaluate patients with physical functioning disorders and wound care needs
• Develop and adjust plans of care based on patient progress and outcomes
• Administer manual techniques, therapeutic exercise, ambulation, modalities, and supportive devices
• Document treatment, progress, and discharge information in the electronic record
• Communicate patient status, barriers, and plans with the care team
• Provide consultation and education to patients, families, and caregivers
• Support physical therapist assistants, rehabilitation aides, and students
