Travel Physical Therapist in Ada, OK

A Travel Physical Therapist will evaluate and treat patients in a long term care unit on a 13 week contract in Ada, Oklahoma. The role uses standardized assessments, evidence-based treatment plans, electronic documentation, and rehabilitation support for patients with physical functioning disorders and wound care needs.

• Screen and evaluate patients with physical functioning disorders and wound care needs
• Develop and update individualized plans of care
• Provide manual techniques, ambulation training, therapeutic exercise, modalities, assistive devices, and wound care support
• Document progress, treatment minutes, and discharge summaries in the record
• Communicate patient progress, barriers, and treatment plans with the care team
• Support patients, families, caregivers, assistants, aides, and students
• Participate in care conferences, utilization review, and rehabilitation meetings

• Oklahoma Physical Therapist license
