Travel Occupational Therapist in Hermiston, OR

A Travel Occupational Therapist will provide in-home occupational therapy services in the home health setting on a 13-week contract in Hermiston, Oregon. It supports older adults and home health and hospice patients in their homes. Interdisciplinary coordination and accurate documentation are central to the role.

• Evaluate functional status and therapy needs
• Complete assessments and reassessments using objective measures
• Develop and implement plans of care with the interdisciplinary team
• Educate patients and coworkers on therapy goals and home strategies
• Document communications, treatment activity, and care coordination accurately
• Adapt to changes in condition and urgent situations
• Support self-care, leisure, and work goals in the home setting

• Active Oregon Occupational Therapist license
• Graduate of an accredited occupational therapy program
• 3 years of occupational therapy experience
• Current BLS certification, valid driver's license, and proof of auto insurance
• Knowledge of physical disability, rehabilitation, mental health, older adults, long-term care, and home health and hospice care

• Professional occupational therapist registration
• Continuing education in occupational therapy practice
