Travel Case Management RN in Port Charlotte, FL

A Travel Case Management Registered Nurse will coordinate discharge planning and utilization review in an acute care hospital for a 13-week contract in Port Charlotte, Florida. The unit uses Meditech, has unit-based RN care managers and social workers, and typically works with a 1:20-25 ratio.

• Coordinate patient discharge planning and transitions of care
• Collaborate with physicians, social work, nursing, and ancillary teams
• Review admissions and length of stay against utilization criteria
• Document case management activities and care plans in Meditech
• Prioritize patient needs and manage caseloads across a busy unit

• Florida Registered Nurse license
• Hospital case management experience
• Accredited nursing program graduate
• Detail-oriented time management and multitasking skills

• InterQual experience
• Meditech experience
