A Travel Case Management Registered Nurse will coordinate care transitions, discharge planning, and utilization review in an acute care hospital on a 13-week contract in Tallahassee, Florida. The role supports a multidisciplinary team and uses MIDAS for case management documentation.

• Coordinate admission reviews and ongoing case management
• Develop discharge plans and transition services
• Collaborate with physicians, nurses, social workers, and community providers
• Monitor utilization, length of stay, and patient progress
• Document case management findings and care plans in MIDAS
• Support patient advocacy and resource planning for complex needs

• Active Registered Nurse license in Florida
• MIDAS experience
• Graduate of an accredited nursing program
