Travel Case Management RN in Jacksonville, FL

A Travel Case Management Registered Nurse will support care coordination and utilization review in an acute care hospital on a 13-week contract in Jacksonville, Florida. The unit uses Meditech and CarePort and works with a 1:21 ratio.

• Perform daily case management assessments and evaluations
• Coordinate discharge planning and transitions of care
• Collaborate with physicians, nurses, and support staff
• Review documentation and update care plans
• Support utilization review and resource coordination
• Communicate patient needs and discharge barriers

• Active Florida Registered Nurse license
• 1 to 2 years of case management experience
• Strong computer and documentation skills

• CCM certification
