A Travel Case Management Registered Nurse will coordinate care and discharge planning for acute care hospital patients on a 13-week contract in Fort Walton Beach, Florida. The role uses Meditech, Midas, and Interqual in a case management unit.

• Coordinate discharge planning and placement needs
• Review admissions, continued stay, and utilization needs
• Collaborate with the care team on transitions
• Document case management activities and outcomes
• Assist with resource coordination and patient education
• Support timely communication across the episode of care

• Florida Registered Nurse license
• Acute care hospital case management experience
• Meditech, Midas, and Interqual experience
