Travel Case Management RN in Fort Pierce, FL

A Travel RN Case Management Registered Nurse will coordinate discharge planning, utilization review, and care transitions in an acute care hospital on a 13-week contract in Fort Pierce, Florida. The setting is a Level II trauma hospital with a fast-paced, high-acuity case management team.

• Coordinate discharge planning and care transitions
• Complete initial and discharge assessments using InterQual criteria
• Collaborate with nursing, social work, and care management team members
• Review utilization needs and support timely discharge planning
• Communicate with patients and families about next steps in care
• Document case management findings in the electronic record

• Florida or compact Registered Nurse license
• 2-4 years of RN case management experience in an acute care facility
• InterQual experience for initial and discharge assessments
• Strong care coordination, utilization review, and discharge planning skills

• Meditech experience
• Prior travel experience
• Ability to work independently in a fast-paced unit
