Travel Case Management RN in Asheville, NC

A Travel Case Management Registered Nurse will coordinate care planning and discharge support in a hospital in Asheville, North Carolina on a 13-week contract. The role supports a 1:40 caseload and uses Cerner and post-acute systems.

• Coordinate care plans with the interdisciplinary team
• Assess discharge needs and barriers to safe placement
• Communicate status updates with patients, families, and staff
• Document case management activity in Cerner and related systems
• Support utilization review and resource planning
• Promote timely transitions across levels of care

• North Carolina Registered Nurse license
• Graduation from an accredited nursing program
• Strong documentation, communication, and teamwork skills
• Ability to manage multiple tasks and priorities
