Travel Case Management RN in Asheville, NC

A Travel Case Management Registered Nurse will coordinate discharge planning and care transitions in a hospital setting on a 13-week contract in Asheville, North Carolina. The care management unit uses Cerner, NATE, and ENSOCARE and works with a 1:40 ratio.

• Coordinate patient discharge plans and care transitions
• Collaborate with nurses, providers, patients, and families
• Document assessments, plans, and updates in the record
• Review service needs and follow-up arrangements
• Identify barriers to timely discharge
• Build trust through clear communication and support

• Active North Carolina Registered Nurse license
• Graduation from an accredited nursing program
• Documentation and care coordination skills
• Strong communication and time-prioritization skills
• Ability to build trust with patients and families
