Travel Social Worker in Medford, OR

A Travel Social Worker Licensed Clinical Social Worker will support behavioral health patients in a short term acute care hospital on a 13 week contract in Medford, Oregon. Epic charting is used, and the setting serves adolescents, adults and geriatrics. First-time travelers are welcome.

• Support behavioral health patients in emergency and inpatient settings
• Complete biopsychosocial assessments and initial evaluations
• Develop and revise care plans and goals with patients and families
• Provide crisis intervention and safety-focused social work support
• Coordinate discharge planning, contingency planning and community resources
• Collaborate with interdisciplinary teams to support continuity of care
• Document assessments, interventions, patient response and care coordination

• Licensed Clinical Social Worker in Oregon
• 2 years of social work experience
• Biopsychosocial assessment, crisis intervention and discharge planning skills
• Experience with behavioral health risk assessment and care planning
• Epic charting system experience

• Emergency department, medical-surgical and psychiatric social work experience
• Involuntary holds, adult protective services and guardianship knowledge
• Advance directives, collaborative care and community resource coordination
