Travel Hospice Palliative Care RN in Newport, VT

A Travel Hospice Palliative Care Registered Nurse will provide home-based nursing care in home health and hospice on a 25-week contract in Newport, Vermont. Home visits and outpatient care are central to the assignment, with OASIS documentation and HealthWyse (Careficient) charting used throughout.

• Visit patients in homes and complete skilled nursing assessments
• Develop and update care plans based on changing patient needs
• Document OASIS data and visit notes in the charting system
• Coordinate care with hospice and home health team members
• Educate patients and families on symptom management and care goals
• Monitor comfort, response to treatment, and safety in the home setting

• Active Vermont Registered Nurse license
• BLS/BCLS certification
• 2 years of home health and hospice experience
• OASIS experience
• Valid driver's license in good standing
