Travel Hospice Palliative Care RN in Grand Junction, CO

A Travel Hospice Palliative Care Registered Nurse will provide compassionate care to patients and families in a community hospice setting on a 12-week contract in Grand Junction, Colorado. Care is delivered in private homes and other residential settings. The role centers on symptom management, caregiver teaching, and coordination with the interdisciplinary team.

• Assess patient symptoms and changing needs
• Develop and update individualized plans of care
• Teach caregivers to provide daily support and comfort measures
• Coordinate with the interdisciplinary team on goals of care
• Document visits, assessments, and care plans
• Support patients in private homes and other residential settings

• Colorado Registered Nurse license upon acceptance
• 2 years of nursing experience
• Strong assessment and critical thinking skills
• Care planning and caregiver education skills
