Travel Hospice Palliative Care RN in Moab, UT

A Travel Hospice Palliative Care Registered Nurse will provide end-of-life nursing care to patients and families in a home-based hospice setting on a 13-week contract in Moab, Utah. The role supports symptom management, family education, and interdisciplinary coordination.

• Assess patients' needs and update individualized care plans
• Manage pain and symptom control within the hospice plan of care
• Educate patients and families on disease process and self-care
• Coordinate care with physicians, social workers, chaplains, and volunteers
• Document care accurately and timely in the chart
• Deliver skilled nursing support to terminally ill patients and families

• Active Registered Nurse license in Utah or a compact state
• Current BLS certification
• Minimum 3 years of nursing experience
• Valid driver's license and reliable transportation
• Strong clinical assessment, communication, interpersonal, and organizational skills

• Hospice, home health, or palliative care experience
• Hospicesoft or Careficient EMR experience
