Travel Case Management RN in Wichita, KS

A Travel Case Management Registered Nurse will provide case management support in an acute care hospital on a 13-week contract in Wichita, Kansas. The assignment includes cardiac and medical step-down units, with Meditech and Midas charting. The team is collaborative and welcoming.

• Complete case management assessments
• Coordinate discharge planning and placement needs
• Collaborate with physicians and mid-level providers
• Support transitions home and to cardiac rehab
• Document care coordination in the electronic record
• Work with patients in cardiac and medical step-down settings

• Active Kansas Registered Nurse license
• BLS certification
• Minimum 1 year of current acute care case manager Registered Nurse experience

• Meditech experience
• Midas experience
