A Travel Physical Therapist will evaluate and treat patients in an outpatient rehabilitation clinic on a 13-week contract in Stockton, California. Electronic medical record charting is used to support care and discharge planning.

• Evaluate and treat patients in rehabilitation programs
• Guide patient care through treatment and follow-up
• Supervise PT Assistants and Aides
• Document evaluations, reports, and discharge summaries
• Communicate with physicians and office staff
• Support patient comfort, safety, and privacy compliance

• Active Physical Therapist license in California or eligibility to obtain it soon
• Degree in Physical Therapy from an accredited program
• 1 year of physical therapy experience
• Electronic medical record documentation skills
• Independent clinical judgment and strong communication skills
