Travel Physical Therapist in Petaluma, CA

A Travel Physical Therapist will provide acute rehabilitation in a hospital physical therapy unit on a 26-week contract in Petaluma, California. The caseload includes adults and may blend inpatient and outpatient care, with Epic charting and a need for independent time management.

• Evaluate mobility, strength, balance, and functional status
• Develop and adjust treatment plans for adult rehab needs
• Provide gait training, therapeutic exercise, and transfer training
• Teach home exercise programs and safe use of assistive devices
• Coordinate discharge planning and patient education with the care team
• Document assessments, interventions, and outcomes in Epic

• California Physical Therapist license
• BLS certification
• 2 years of acute care physical therapist experience
• Inpatient and outpatient adult rehabilitation experience
• Epic charting system experience
• Graduation from an accredited Physical Therapy program
• Strong time management and independent work skills

• Community hospital experience
• Cardiopulmonary rehabilitation experience
• Orthopedic, neurologic, and post-operative rehabilitation experience
• Wheelchair assessment and specialty bed experience
