Travel Physical Therapist in Alamosa, CO

A Travel Physical Therapist will provide outpatient rehabilitation care in an acute care hospital on a 13-week contract in Alamosa, Colorado. The caseload includes adolescents, adults, and geriatrics, with one half day of inpatient evaluation coverage. Charting uses OBix, McKesson, Pyxis, and SBAR.

• Evaluate outpatient and inpatient referral patients
• Perform physical therapy assessments and reassessments
• Develop plans of care and home exercise programs
• Provide gait training and functional mobility treatment
• Educate patients and families on recovery and safety
• Document progress and coordinate discharge planning
• Collaborate with physicians and care teams

• Colorado Physical Therapist license or compact license
• BLS certification
• 2 years of physical therapy experience
• Graduation from an accredited physical therapy program

• ACLS certification
• Acute care and outpatient rehabilitation experience
• 5 years of inpatient experience
• Charting system experience
