Travel Occupational Therapist in Santa Rosa, CA

A Travel Occupational Therapist will provide rehabilitation care to residents in a nursing home on a 13-week contract in Santa Rosa, California. The role supports post-acute and long-term care in a resident-focused setting with an interdisciplinary team and modern therapy equipment.

• Evaluate residents and develop individualized treatment plans
• Deliver therapeutic interventions to improve function, ADLs, and safety
• Collaborate with nursing and rehabilitation staff on care goals
• Maintain documentation in compliance with state and federal requirements
• Support residents in regaining independence and daily living skills

• Current California Occupational Therapist license
• Minimum 1 year of occupational therapy experience
• Strong interpersonal and clinical skills

• Skilled nursing or post-acute experience
