A Travel Care Coordination Registered Nurse will direct care coordination in an acute-care hospital on a 13-week contract in Greenbrae, California. The role supports utilization management, discharge planning, patient progression, and transitions of care. It also leads interdisciplinary programs and performance improvement.

• Lead care coordination, utilization management, and discharge planning
• Oversee patient progression and transitions of care
• Direct interdisciplinary clinical and operational programs
• Supervise staff and support daily operations
• Drive performance improvement and process development
• Maintain readiness for regulatory and accreditation review

• Active California Registered Nurse license
• Bachelor of Science in Nursing
• BLS/BCLS
• 5 years of acute healthcare experience
• 5 years of leadership or administrative experience in case management, care coordination, utilization management, or a closely related function
• Advanced knowledge of case management, discharge planning, patient progression, and transitions of care

• Master of Science in Nursing, Health Administration, or Business Administration
• Case Management certification such as CCM or ACM
