Travel Case Management RN in Portsmouth, NH

A Travel Case Management Registered Nurse will coordinate patient care in an acute care hospital for a 13-week contract in Portsmouth, New Hampshire. The unit uses Midas, has an 18:1 patient ratio, and focuses on utilization management, care coordination, discharge planning, and revenue integrity.

• Coordinate patient care across the hospital
• Support utilization management reviews
• Plan safe and timely discharges
• Collaborate with providers and care teams
• Track documentation and case status in Midas
• Help align care plans with revenue integrity goals

• Active New Hampshire Registered Nurse license
• Active compact Registered Nurse license
• BLS certification
• 2 years of hospital experience
• Acute hospital case management experience
• Critical care experience
