A Travel RN Case Management Registered Nurse will coordinate care, utilization review, and discharge planning in a long-term acute care hospital on a 12-week contract in Los Angeles, California. The role supports ongoing assessment, care coordination, and interdisciplinary planning for patients with complex needs.

• Coordinate care plans and resource use
• Perform utilization review and discharge planning
• Assess patient needs and update care plans
• Collaborate with the interdisciplinary team
• Advocate for patient preferences and continuity of care
• Support compliance with review requirements
• Evaluate outcomes and adjust plans as needed

• California Registered Nurse license
• Basic Life Support certification through the American Heart Association
• 2 years recent RN Case Management experience
• LTAC or STACH experience
• Discharge planning across acute care settings
• Strong interdisciplinary team collaboration skills
