Travel Case Management RN in Apple Valley, CA

A Travel Case Management Registered Nurse will support discharge planning and care coordination in an acute care hospital on a 13-week contract in Apple Valley, California. The role uses Epic and serves neonates through geriatrics with a caseload up to 25 patients. It coordinates with physical therapy and social services for complex placement needs.

• Assess patient needs and discharge barriers
• Coordinate care plans with the interdisciplinary team
• Complete utilization review and authorization tasks
• Arrange DME, SNF placement and ambulance transport
• Lead family meetings and support treatment planning
• Document case management activities in Epic

• California Registered Nurse license
• 2 years of acute care case management and discharge planning experience
• Epic charting system experience
• Prior travel experience
• Care coordination, discharge planning, utilization review and prior authorization skills
• DME planning, placement coordination and transport arrangement experience
