Travel Ambulatory Care RN in Farmington, CT

A Travel Ambulatory Care Registered Nurse will support outpatient nephrology care in a hospital clinic on a 13-week contract in Farmington, Connecticut. The setting uses Epic charting and focuses on nephrology patients in an ambulatory environment.

• Assess outpatient nephrology patients and review symptoms, histories, and treatment plans
• Coordinate clinic visits, labs, and follow-up care with providers and care teams
• Administer and monitor ordered therapies within the ambulatory setting
• Document assessments, interventions, and patient teaching in Epic
• Educate patients on kidney disease management, medications, and self-care
• Support care transitions and respond to changes in patient condition

• Connecticut Registered Nurse license
• Basic Life Support certification
• Epic charting experience
• 2 years of acute care nursing experience

• Experience with chronic kidney disease
